29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA. Establishment - The employer did not notify OSHA within twenty-four (24) hours after the in-patient hospitalization of one of their employees. The Portsmouth Naval Shipyard was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1904.39(a)(2), which was contained in OSHA inspection number 1075896, citation number 1, item number 1 and was affirmed as a final order on 09/29/2015, with respect to a workplace located at Portsmouth Naval Shipyard.
