29 CFR 1910.147(a)(2)(ii): Alternative measures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by the exception of 1910.147(a)(2)(ii) for routine, repetitive, and integral processes such as tool change, change over, and cleaning.    ESTABLISHMENT:  The employer did not develop, document and utilize alternative measures for the control of potentially hazardous energy where employees were required to perform routine set up and/or cleaning for machines, such as (but not limited to) Premier Die Cutter Machine #140, and Premier Die Cutter Machine #155 while the machines were still energized.    On or about 09/26/2016, while an employee was trying to reach into the clutter area of the Premier Die Cutter Machine #140 to clean pieces of cardboard scarp without shutting down the machine, the employee's both hands got caught into the cutter, resulting amputations to fingers on both hands.
