On or about September 18, 2016 and at times prior, at the manlift, tools and machine parts larger than what would fit entirely in a work clothing pocket were carried by employees on the manlift restricting the ability to maintain balance and a secure handhold, exposing employees to falls greater than 4 feet to a lower level.

29 CFR 1910.68(b)(12): Emergency exit ladder. A fixed metal ladder accessible from both the "up" and "down" run of the manlift was not provided for the entire travel of the manlift. Such ladder was not in accordance with the existing ANSI A14.3-1956 Safety Code for Fixed Ladders and 1910.23. a. On or about September 18, 2016 and at times prior thereto, at the manlift, the fixed ladder was not in accordance with 29 CFR 1910.23 in that the perpendicular distance from the centerline of the rungs to the nearest permanent object on the climbing side of the ladder was closer than 30 inches. Employees are exposed to falls, and/or caught-by hazards.
