29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: a) On or about September 28, 2016 an employee was installing roofing material on a one story single family house at a height of approximately 8-feet 7-Inches to ground, was using a harness with life line and lanyard for fall protection and his lanyard was connected to the front of the harness instead of connecting to the attachment point located on the back of the body harness.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support). a. On or about September 27, 2016, employees were using a Werner extension ladder that was not adequately installed to access the roof, exposing employees to a fall hazard. b. On or about September 28, 2016 employees were using a Werner extension ladder that was not adequately installed to access the roof, exposing employees to a fall hazard.
