29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours. a) Whitesell-Green, Inc. Norfolk Naval Shipyard Portsmouth: On or about 09/22/2016, the employer did not report within 24 hours that an employee's left index finger was amputated at the tip. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
