29 CFR 1910.132(d)(1)(i): The employer did not have each affected employee use the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment;    106 Lonczak Drive, Chicopee, MA    On September 21, 2016 and at times prior, some metal finishing employees did not wear protective aprons when operating machines with hot liquid burn hazards.  On September 21, 2016, a metal finishing employee, who was not wearing a protective apron, received second degree burn injuries when operating a Toho Koki 30L Centrifugal Barrel Polisher Machine.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard were being followed.    106 Lonczak Drive, Chicopee, MA    As of November 14, 2016, at the closing of the inspection, the employer did not ensure an annual inspection of the energy control procedure was performed by an authorized employee.  The employer did not ensure the inspection included: a review of employee responsibility between the authorized employee and each authorized employee and a certification by the employer identifying the machine or equipment on which the energy control procedure was utilized, the date of the inspection, the employees included in the inspection and the person performing the inspection.
