29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:   (a)  On or about October 3, 2016, during the installation of roofing material at a job site located at 8606 Gentry St NW, Massillon, Ohio 44646, the employer did not ensure employees working on the North and South sides of a roof of a single family home were protected from a fall of approximately 20.6 feet by utilizing a fall protection sytem.

On or about October 3, 2016 during the installation of roofing material at the job site located at 8606 Gentry St NW, Massillon, Ohio 44646; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material utilizing air driven nail guns.
