29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 1 for eye/face hazards and lack of eye protection, Citation 1 Item 2 for fall hazards and lack of fall protection systems/devices, and Citation 1 Item 3 for fall hazards and not extending the ladder at least three feet above the upper landing surface.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about July 23, 2019, employees were exposed to a struck by hazard while performing roofing activities on a residential home located at 115 Greencrest Terrace, Akron, Ohio 442313. The employees were using air powered nail guns without wearing protective safety glasses.  One Stop Construction Shop LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.102, which was contained in OSHA inspection number 1181145 citation number 1,  item number 1 and was affirmed as a final order on 12/21/2016, with respect to a workplace located at 8606 Gentry St NW, Massillon, Ohio 44646.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about July 23, 2019, employees were exposed to a fall hazard while performing roofing activities on a residential home located at 115 Greencrest Terrace, Akron, Ohio 442313. The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.    One Stop Construction Shop LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 1181145 citation number1,  item number 2 and was affirmed as a final order on 12/21/2016, with respect to a workplace located at 8606 Gentry St NW, Massillon, Ohio 44646.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about July 23, 2019, employees were exposed to a fall of approximately 8 feet while performing roofing activities on a residential home located at 115 Greencrest Terrace, Akron, Ohio 442313. The employees were using an extension ladder that did not extend at least three feet above the upper landing area.   One Stop Construction Shop LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1181145 citation number 1,  item number 3 and was affirmed as a final order on 12/21/2016, with respect to a workplace located at 8606 Gentry St NW, Massillon, Ohio 44646.
