29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    a. Spray Area: On or about October 3, 2016, and at times prior, employees were required to wear 3M Half Mask Elastomeric respirators with combination cartridges without having had a medical evaluation prior to use.     b. Spray Area: On or about October 3, 2016, and at times prior, employees were required to wear 3M Half Mask Elastomeric respirators with combination cartridges without having had a fit test prior to use.     c. Spray Area: On or about October 3, 2016, and at times prior, employees were required to wear 3M Half Mask Elastomeric respirators with combination cartridges without having training in respiratory hazards to which they would potentially be exposed on a routine basis including, but not limited to, n-butyl acetate, 1-butanol, and ethanol contained in Enviromax C130644 Pre Cat Lacquer, and proper use, limitations, and maintenance of the respirators.

Main Shop: On or about October 3, 2016, and at times prior, employees were exposed to lacerations and amputations from working on an unguarded Holzher Table Saw.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:    a. Main Shop: On or about October 3, 2016, and at times prior, employees were exposed to a hazardous release of air, flying debris, and air embolisms when using compressed air from an Atlas Copco GX11p air compressor measured at greater than 100 psi to blow off their workstations and clothes.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    a. Square One Cabinets: On or about October 3, 2016, and at times prior, employees were exposed to chemical hazards including, but not limited to, unknown chemicals being stored in PowerAde bottles and unknown chemicals being stored in unlabeled containers.      b. Square One Cabinets: On or about October 3, 2016, and at times prior, employees were exposed to chemical hazards from spraying Wilsonart 950 Series Spray Adhesive, and Enviromax C130644 Pre Cat Lacquer, which contains n-butyl acetate, ethanol and 1-butanol, without information and training regarding the hazards associated with these, and other, chemicals throughout the shop.
