The employees were not protected from falling to the floor below.

The Employee was approximately 24 feet above the ground and was not protected from falling.

29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself. Location: Roof access stairway ; 1417 Longfellow Ave. BRONX, NY, 10459 On or about: 10/3/16 a) Improper use of shock absorber lanyard; the lanyard was wrapped around a horizontal I-beam. The snaphook connector was directly engaged to rope, which is unsuited per the manufacture's designed application.
