29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  Location: 3rd floor, building's East face at 1417 Longfellow Ave. Bronx NY 10459  On or about: 10/3/16  a) Employees were engaged in preparation of wall opening work, approximately 10 feet above the lower level. The employees were not protected from falling to the floor below.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   Location: 3th floor, North East corner of building at 1417 Longfellow Ave. Bronx NY 10459 On or about: 10/3/16  a) An employee stepped out of wall opening and climbed onto 4th tier scaffold. The Employee was approximately 24 feet above the ground and was not protected from falling.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Location:  Elevator bulkhead roof; 1417 Longfellow Ave. Bronx, NY  On or about 10/3/2016  a) Employees were installing insulation on the elevator bulkhead roof, approximately 18 feet above the lower level. The employees were not protected from falling to floor below.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.   Location: 3th floor, 1417 Longfellow Ave. Bronx NY 10459 On or about: 10/3/16  a) Employees were engaged in preparing wall openings for installing air conditioners and windows. Employees were approximately 30 feet above the ground and were not protected from falling.

29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.  Location:  Roof access stairway ; 1417 Longfellow Ave. BRONX, NY, 10459  On or about: 10/3/16  a) Improper use of shock absorber lanyard; the lanyard was wrapped around a horizontal I-beam. The snaphook connector was directly engaged to rope, which is unsuited per the manufacture's designed application.
