Selmco Metal Fabricators, Inc. located in Sidney, Ohio: On or about September 29, 2016, the employer did not develop and implement a lockout program containing energy control procedures, employee training, and periodic audits. Employees are exposed to the unexpected start-up of equipment such as the hydraulic press brakes during set-up operations.

The guarding device was not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, was not designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. Selmco Metal Fabricators, Inc. located in Sidney, Ohio: On or about September 29, 2016, the employer did not ensure employees operating the Cincinnati 400H hydraulic press brake were protected from point of operation hazards created by the closing of the top and bottom dies. b. Selmco Metal Fabricators, Inc. located in Sidney, Ohio: On or about September 26, 2016, the employer did not ensure employees operating the Amada D9E FBD3512 hydraulic press brake were protected from point of operation hazards created by the closing of the top and bottom dies.
