29 CFR 1926.451(b)(4): Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches (15 cm).  a.  On or about October 4, 2016, at the above addressed jobsite, employees were exposed to falls while performing masonry operations from a tubular welded frame scaffold, appoximately twenty-five (25) feet above the ground, that did not have the planks extended over six (6) inches or cleated to prevent displacement.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph.  a. On or about October 4, 2016, at the above addressed jobsite, employees were performing masonry operations  from a tubular welded frame scaffold, approximately twenty-five (25) feet above the ground, that was not restrained from tipping, thereby exposing employees to falls.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a. On or about October 4, 2016, at the above addressed jobsite, employees performing masonry operations from a tubular welded frame scaffold, greater than twenty-five (25) feet above the ground, were exposed to injuries from falls due to the lack of guardrails.
