29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) Facility: On or about October 3, 2016, the one Clark forklift had one hole near the end of each fork. The other Clark forklift had a hole near the end of one of its forks. The company punched these holes in the ends of the forks so that the forklifts could move material/machinery (i.e. welders) around.
