29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment for the use of personal protective equipment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment.    On or about October 4, 2016, the employer did not evaluate, develop and document a personal protective equipment hazard assessment for the CNC plasma cutting tasks performed, nor did they complete a written certification that included the name of the person who certified that the evaluation had been performed and the date that the hazard assessment was completed.

On or about October 4, 2016, an employee was exposed to eye injury from ultraviolet light emissions while the CNC plasma cutter is operated and the employer did not require the following:  a) shaded safety glasses for the CNC plasma cutter;  b) screens/curtains for the adjacent employees working at the two brakes and saw.

29 CFR 1910.133(a)(3): The employer did not ensure that employees wearing prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design or wears eye protection that can be worn over the prescription lenses.  On or about October 4, 2016, an employee was exposed to eye injury and the employer did not require safety glasses that incorporated prescription lenses when cleaning metal pieces by chipping and operating the handheld Dewalt grinder.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:    On or about October 4, 2016, employees were exposed to fire and smoke inhalation hazards and the fire extinguisher(s) was:    a) blocked by the Millermatic 125 adjacent to the Operators desk at the CNC plasma cutting table; and    b) blocked by scrap metal and a ladder near the entrance from the CNC plasma cutting area to the welding shop.

There was no written documentation that included the operator's name.  MAST PRODUCTSIONS, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATINAL SAFETY AND HEALTH STANDARD, WHICH WAS CONTAINED IN INSPECTION 929668, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON NOVEMBER 13, 2013.
