29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  (a) Northern Roofers on or about October 05, 2016, at the address of 228 E. South St. Carey, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) Northern Roofers on or about October 05, 2016, at the address of 228 E. South St. Carey, OH: The employer did not ensure employees were protected from a falls while working from ladder jack scaffolding system.  Two employees installing roofing material from the scaffold at the west side of the structure in this condition were exposed to falls up to 18 to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  Northern Roofers on or about October 05, 2016, at the address of 228 E. South St. Carey, OH: The employer did not ensure employees were protected from falls while installing new roofing material to a residential type structure.  Employees operating in this condition were exposed up to a 20 ft. fall to the ground below.

South St. Carey, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing various roofing operations from elevated surfaces.
