Vance Street, Carey, OH: The employer did not have a plan or program to identify job site hazards which assist employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

Vance Street, Carey, OH: The employer did not ensure each operator operating the forklift was trained on the powered industrial vehicle. The exposed employee was exposed to struck by hazards while being lifted in an elevated work platform on the forks of the powered industrial truck, without the benefit of operator training.

Vance Street, Carey, OH: The employer did not ensure the scaffolding platform used to elevate employees via forklift, was designed by a qualified person. Employees performing construction activities in this condition were exposed to a fall of approximately 18 feet.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level.  a)  On or about March 11, 2020, at work site located at 317 N. Vance Street, Carey, OH: the employer did not ensure employees were protected from fall hazards while riding on the platform to perform work to the edge of the roof.  Employees were exposed to an 18 fall hazard in that no front guardrails or fall protection were used while employees were lifted by the JLG Sky Trak 6036 rough terrain fork lift.

Vance Street, Carey, OH: The employer failed to ensure that all employees were protected from falls while performing roofing work on residential structure. The individual was observed sheathing the roof without the benefit of any type of fall protection.
