Vance Street, Carey, OH: the employer did not ensure employees were protected from fall hazards while riding on the platform to perform work to the edge of the roof. Employees were exposed to an 18 fall hazard in that no front guardrails or fall protection were used while employees were lifted by the JLG Sky Trak 6036 rough terrain fork lift.
