29 CFR 1926.451(e)(9)(i): The employer did not provide safe means of access for each employee erecting or dismantling a scaffold: (a) Main Street, Exterior: On or about October 5, 2016, during the erection process, employees used the scaffold frame and cross-bracing as a means of access, ascending as high as approximately 14 feet on the day of the inspection. Casey Enterprise, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 976650, citation number 1, item number 1a and was affirmed as a final order on 09/23/2014, with respect to a workplace located at Peyton Street, Barboursville, WV 25504.

29 CFR 1926.454(b): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work: (a) Main Street, Exterior: On or about October 5, 2016 employees were assisting in the erection of a tubular welded scaffold and had not been trained by the employer to recognize the associated hazards or the procedures to minimize those hazards. Casey Enterprise, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.454(a) which was contained in OSHA inspection number 976650, citation number 1, item number 1b and was affirmed as a final order on 09/23/2014, with respect to a workplace located at Peyton Street, Barboursville, WV 25504.
