29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (a) Supply Network, Inc. dba Viking Supply Net at 12360 E 46th Avenue, Denver, CO 80239: On and preceding 9/30/16, the employer did not ensure that employees wearing a tight-fitting face piece did not have facial hair that came between the sealing surface of the face piece and the face. Two out of six welders onsite had long beards while wearing a half-mask respirators during welding operations. This condition potentially exposed the employees to a respiratory hazard.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of this section, in any written or oral format to employees who wear respirators when such use is not required by this section or by the employer: (a) Supply Network, Inc. dba Viking Supply Net at 12360 E 46th Avenue, Denver, CO 80239: On and preceding 9/30/16, the employer did not provide the basic advisory information on respirators, as presented in Appendix D of this section, in any written or oral format to employees who wear respirators when such use is not required by this section or by the employer. Employees were allowed to wear respiratory protection with beards when conducting welding operations. The employer had not provided the basic advisory information on respirators to all employees who voluntarily wore respiratory protection. This condition potentially exposed the employees to a respiratory hazard.
