On or about October 6, 2016, procedures, including the use of locks and blocking devices, were not utilized during die changing operations on machinery including, but not limited to, the Niagara 150 Ton Auto Tool Press.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points, rotating parts, flying chips and sparks: a) Connex, Inc., 7660 Kier Dr. in Fairview, PA. On or about October 6, 2016, the back of the Cincinnati 375 Hydraulic Shear was not guarded to prevent employees from being struck by the rear bar of the shear during operating. b) Connex, Inc., 7660 Kier Dr. in Fairview, PA. On or about October 6, 2016, the hold-downs of the shear were not guarded to prevent the operator from having his fingers crushed by the hold-downs while operating the Cincinnati 375 Hydraulic Shear. c) Connex, Inc., 7660 Kier Dr. in Fairview, PA. On or about October 6, 2016, the rotating spindles that held the rolls of sheet metal for the N. Ferrarra Edger were not guarded to prevent operators from being caught-by the rotating parts during operation. d) Connex, Inc., 7660 Kier Dr. in Fairview, PA. On or about October 6, 2016, the ingoing nip points of the N. Ferrarra Edger were not guarded. The nip points were located where the sheet metal is fed into the edger.
