29 CFR 1910.106(e)(2)(iv)(d): Flammable liquids were not drawn from or transferred into vessels, containers, or portable tanks within a building only through a closed piping system, from safety cans, by means of a device drawing through the top, or from a container or portable tanks by gravity through an approved self-closing valve:   a.)  Connex, Inc., Area outside of the tumbling room, on or about October 17th, 2017 and times prior there to:  Employees were dispensing a flammable liquid (Beacon brand Anti-Rust 165) during tumbling operations of parts.  The dispensing valve for the flammable liquid was not self-closing.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:      a.)  Connex, Inc., Eyewash station outside of tumbling room, on or about October 17th, 2017 and times prior there to:  Employees worked with and around corrosive chemicals including, but not limited to, Chemetall Product 317 and Beacon lubricants Anti-Rust 165 during tumbling operations at the facility.  The Sperian brand (non-pressurized self-contained eyewash) outside of the tumbling room was not in compliance with ANSI/ISEA 358.1-2014 for Emergency Eyewash and Shower Equipment in that:    1)  Weekly visual inspections of the eyewash station were not conducted. The emergency eyewash unit was not visually inspected weekly to verify that the flushing fluid does not need to be changed or supplemented.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:    a)  Connex, Inc., Niagara #300-05 Press area, on or about October 17th, 2017 and times prior there to:  An approximately 32 ounce secondary container of Condat brand DP-EXP lubricant was not labeled with a product identifier nor with the correct hazard label associated with the chemical(s) contained therein.
