29 CFR 1910.333(c)(3)(iii)(A): Vehicles or mechanical equipment capable of having parts of their structures elevated near energized overhead lines were not operated so that a clearance of 10 feet (305 cm) was maintained: On or about September 14, 2016 an employee in the bucket of a boom truck was accessing a roof top to prep the area for painting and he was exposed to electrical shock because the bucket of the boom truck was operated less than 10 feet from energized overhead power lines.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: On or about September 14, 2016 an employee working near power lines was hospitalized after the bucket of the boom truck that he was working from came into contact with over head power lines and the employer did not report the incident.
