29 CFR 1910.333(c)(3)(iii)(A): Vehicles or mechanical equipment capable of having parts of their structures elevated near energized overhead lines were not operated so that a clearance of 10 feet (305 cm) was maintained: On or about June 16, 2016 employees in the basket of an aerial lift power washing a grain storage structure were exposed to electrical shock because the basket of the lift was operated less than 10 feet from energized overhead power lines that had a line voltage of 72kV.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: On or about June 16, 2016 two employees working near power lines were hospitalized after the the basket of the aerial lift that they were working from came into contact with the power lines and the employer did not report the incident until October 20, 2016.
