29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times: (a) In the hall near the bathrooms, the exit route was not adequately lighted in the event of a power outage as the only illumination battery backup lighting was not working properly. When tested using the test switch, the lights did not operate. (b) Emergency lighting in the bathrooms was not working properly. When tested using the test switch the lights did not operate.

The most recent visual inspection was unknown. (b) The Buckeye fire extinguisher in GSA Truck #955, was not visually inspected at least monthly. The extinguisher was covered in dust and the most recent visual inspection was unknown nor was it noted on the vehicle inspection. (c) The Buckeye fire extinguisher in GSA Truck #425 was not visually inspected at least monthly. The extinguisher was covered in dust and the most recent visual inspection was unknown nor was it noted on the vehicle inspection. (d) The Badger fire extinguisher in Engineer's GSA Truck was not visually inspected at least monthly. The most recent visual inspection was unknown nor was it noted on the vehicle inspection.
