29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:   (a) On October 8, 2016, at the south side of the project, a twelve foot fiberglass stepladder was used in the folded position and leaned up against exterior wall supports to access the horizontal steel rails and frames.   Isagani B. Baluyut Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(4), which was contained in OSHA inspection number 1075396, citation number 2, item number 1 and was affirmed as a final order on September 16, 2015, with respect to a workplace located at the Bench retail store renovation project Micronesia Mall, Marine Corps Drive, Dededo, GU 96929.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:   (a) On October 11, 2016, the path to ground on the 50-foot yellow triple outlet extension cord, used to supply power to a the Milwaukee SDS Max Demolition Hammer was not permanent and continuous.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:   (a) On October 11, 2016, at the north side of the project, one of four wheel casters were not locked on the tubular welded frame mobile scaffold.  The platform was about 10 feet, 4 inches above the concrete floor.
