29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.  COMPRESSED GAS STORAGE AREA:  On or about, and at times prior to, October 11, 2016, a 56-inch oxygen cylinder was stored 18 inches from one propane cylinder and 32 inches from a cage containing another propane cylinder.  There was no non-combustible barrier separating the cylinders.

29 CFR 1910.1026(j)(1)(i): The employer did not ensure that all surfaces were maintained as free as practicable of accumulations of chromium (VI):  EMPLOYEE BREAK ROOM:  On or about November 3, 2016, wipe samples were collected on the countertop by the coffee machine and a table in the employee break room and submitted to the laboratory for analysis of hexavalent chromium (Cr6+).  The analytical results indicated that the surfaces were contaminated with Cr6+ thereby exposing all employees to the chemical.
