29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) Jobsite: 3rd and 4th levels, north east end - Employees were exposed to falls hazards while conducting framing activities in proximity to floor holes without covers or guardrail systems erected around them. The 3rd level floor hole measured 2' 10" x 7' 2", with a fall distance of 10' 5", and the 4th level floor hole measured approximately 2' 10" x 3' 7", with a fall distance of 20' 10" (to 2nd level). Noted on or about 10/6/16.

29 CFR 1926.502(b)(4): When the 200 pound (890 N) test load specified in paragraph (b)(3) of this section was applied in a downward direction, the top edge of the guardrail deflected to a height less than 39 inches (1.0 m) above the walking/working level. a) Jobsite: 2nd level, north east end - The top edge of the guard rail erected on the east end of the floor hole deflected to 25" from the walking/working level exposing employees conducting framing activities to a 24' fall hazard. Noted on or about 10/6/16.
