29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:          a) Spray Booth and Drying Area: On or about October 5, 2015, and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to; n-butyl acetate, ethanol, acetone, butanol, propanol, formaldehyde (Sher-Wood Hi Bild Pre-Cat Lacquer). while using these chemicals in various phases of the wood treatment process. The employer did not provide medical evaluations to employees required to use the respirators as required in 1910.134(c)(1)(ii).        b) Spray Booth and Drying Area: On or about October 5, 2015, and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to; n-butyl acetate, ethanol, acetone, butanol, propanol, formaldehyde (Sher-Wood Hi Bild Pre-Cat Lacquer), while using these chemicals in various phases of the wood treatment process. The employer did not provide fit testing procedures for tight-fitting respirators to employees required to use the respirators as required in 1910.134(c)(1)(iii).        c) Spray Booth and Drying Area: On or about October 5, 2015, and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to; n-butyl acetate, ethanol, acetone, butanol, propanol, formaldehyde (Sher-Wood Hi Bild Pre-Cat Lacquer), while using these chemicals in various phases of the wood treatment process. The employer did not provide procedures for proper use of respirators in routine and reasonably foreseeable emergency situations for tight-fitting respirators to employees required to use the respirators as required in 1910.134(c)(1)(iv).        d) Spray Booth and Drying Area: On or about October 5, 2015, and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to; n-butyl acetate, ethanol, acetone, butanol, propanol, formaldehyde (Sher-Wood Hi Bild Pre-Cat Lacquer), while using these chemicals in various phases of the wood treatment process. The employer did not provide procedures for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators of employees required to use the respirators as required in 1910.134(c)(1)(v).        e) Spray Booth and Drying Area: On or about October 5, 2015, and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to;n-butyl acetate, ethanol, acetone, butanol, propanol, formaldehyde (Sher-Wood Hi Bild Pre-Cat Lacquer), while using these chemicals in various phases of the wood treatment process. The employer did not provide procedures to ensure adequate air quality, quantity, and flow of breathing air for atmosphere-supplying respirators for employees required to use the respirators as required in 1910.134(c)(1)(vi).        f) Spray Booth and Drying Area: On or about October 5, 2015, and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to; n-butyl acetate, ethanol, acetone, butanol, propanol, formaldehyde (Sher-Wood Hi Bild Pre-Cat Lacquer), while using these chemicals in various phases of the wood treatment process. The employer did not provide training of employees in the respiratory hazards to which they are potentially exposed during routine and emergency situations for employees required to use the respirators as required in 1910.134(c)(1)(vii).        g) Spray Booth and Drying Area: On or about October 5, 2015, and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to; n-butyl acetate, ethanol, acetone, butanol, propanol, formaldehyde (Sher-Wood Hi Bild Pre-Cat Lacquer), while using these chemicals in various phases of the wood treatment process. The employer did not provide training on putting on, removing, or the limitations of the respirators for employees required to use the respirators as required in 1910.134(c)(1)(viii).        h) Spray Booth and Drying Area: On or about October 5, 2015, and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to; n-butyl acetate, ethanol, acetone, butanol, propanol, formaldehyde (Sher-Wood Hi Bild Pre-Cat Lacquer), while using these chemicals in various phases of the wood treatment process. The employer did not establish work-site specific procedures to regularly evaluate the effectiveness of the program as required in 1910.134(c)(1)(ix).

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:     a. Spray Area: On or about October 5, 2016, and at times prior, employees were exposed to chemical hazards while working with chemicals including, but not limited to, Sher Wood Hi Bild Pre Cat Lacquer. The employer failed to ensure that each container of hazardous chemicals in the workplace was labeled with the product identifier and words, pictures, symbols, or combination thereof, which provides at least general information regarding the hazards of the chemicals.     b. Cabinet Shop: On or about October 5, 2016, and at times prior, employees were exposed to chemical hazards while working with chemicals including, but not limited to, lacquer thinner, and Minwax Wood Finish. The employer failed to ensure that each container of hazardous chemicals in the workplace was labeled with the product identifier and words, pictures, symbols, or combination thereof, which provides at least general information regarding the hazards of the chemicals.
