29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Work site was located at 4102 Bergen Turnpike, North Bergen, NJ 07047. An employee was exposed to amputation hazards as The Holmatic Deli Cup Machine did not have Lock Out / Tag Out procedures that were effectively communicated and understood by the employee using the machine resulting in a significant injury. Violation occurred on or about 9/19/2016. The Colonna Brothers, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i), which was contained in OSHA inspection number 1123601, citation number 1, item number 3 and was affirmed as a final order on 7/6/2016 with respect to a workplace located at 1615 51st Street, North Bergen, NJ 07047.
