29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) Work site was located at 4102 Bergen Turnpike, North Bergen, NJ 07047. Employees were exposed to amputation hazards in that they were not trained in recognition of applicable hazardous energy sources on the Holmatic Deli Cup Machine that did not have Lock Out / Tag Out procedures effectively communicated and understood by employees using the machine resulting in a significant injury. Violation occurred on or about 9/19/2016.
