Employees were exposed to amputation hazards in that they were not trained in recognition of applicable hazardous energy sources on the Holmatic Deli Cup Machine that did not have Lock Out / Tag Out procedures  effectively communicated and understood by employees using the machine resulting in a significant injury.    Violation occurred on or about 9/19/2016.
