29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by a pinch point and a crushing points: (a) 101 Fry Drive, Mechanicsburg Pennsylvania, Stacker - Employees reach into the stacker from the side. The employer did not ensure that the light curtain prevented employees from reaching in through the side while the machine was operating, exposing them to pinch point hazards, on or about October 11, 2016. (b) 101 Fry Drive. Mechanicsburg Pennsylvania, Bundler - Employees reach into the bundler from the side. The employer did not ensure that the light curtain prevented employees from reaching in through the side while the machine was operating exposing them to crushing hazards, on or about October 11, 2016. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA: (a) 101 Fry Drive, Mechanicsburg Pennsylvania - An employee suffered an amputation to the middle finger tip on the left hand, on August 26, 2016. The employer failed to report this work-related amputation to OSHA within 24 hours. (The employer reported this amputation to OSHA on October 4, 2016.) No abatement certification or documentation required.
