29 CFR 1904.39(a)(2): The employer failed to report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: (a) Fry Communications, Incorporated - An employee sustained an amputation of their right pinkie finger on March 18, 2018 and the employer did not report the event to OSHA until April 16, 2018. FRY COMMUNICATIONS, INCORPORATED WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH ADMINISTRATION STANDARD 29 CFR 1904.39(a)(2) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1182907, CITATION NUMBER 2, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON DECEMBER 31 2016, WITH RESPECT TO A WORKPLACE LOCATED AT 800 WEST CHURCH STREET, MECHANICSBURG, PA 17050. No abatement certification or documentation required.
