29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Mill Products, northeast, Behringer horizontal bandsaw no. D1: on or about September 30, 2016, operators were assigned to change band saw blade when the hazardous energy control (lockout tagout) machine-specific procedure was not developed, documented and utilized in the workplace. Operators were exposed to machine caught-in hazards likely to result in serious injury.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    Mill Products, northeast, Behringer horizontal bandsaw no. D1 - on or about September 30, 2016, employees performing servicing tasks (replacing band saw blade) were not required to apply energy controls in the following sequence prior to starting work:    (a) prepare for shut down by recognizing energy hazards and its controls,  (b) turn off and shut down the machine,  (c) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (d) affix lockout or tagout devices to each energy isolating device,  (e) render safe all potentially hazardous stored or residual energy,  (f) verify that isolation and deenergization of the machine or equipment have been accomplished.    Employees performing servicing tasks had their fingers and hands exposed to caught-in hazards when replacing and adjusting band saw blade. Hazardous energy controls (lockout) were not implemented and performed to protect employees from the caught-in hazard, likely to result in amputation. Use of the stop button and/or emergency stop device (alternative measures) did not provide effective protection against unexpected start-up and/or hazardous energy release.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Mill Products, northeast, Behringer horizontal bandsaw no. D1: on or about September 30, 2016, operators were cutting ends of steel stock bundles where the unused portion of a running blade was not guarded to prevent access. Provided physical barrier guards were damaged, bent, and misaligned allowing for access to danger zone. Installed limit switches on enclosures were not functioning as designed and constructed (i.e. by-passed and sticking). Operators were exposed to machine caught-in hazards likely to result in serious injury.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
