29 CFR 1926.1053(a)(1)(i): Self supporting portable ladders were not capable of supporting, without failure, at least four times the maximum intended load:  a)  At 7943 Oakcrest Place, Montgomery, Alabama:  On or about October 12, 2016 and at times prior; the employer allowed his employees to use a job made ladder to gain access to the roof and the ladder did not have braces installed between the ladder rungs exposing the employee to a fall hazard of 11 feet to the floor below.

29 CFR 1926.1053(a)(3)(i): Rungs, cleats, and steps of fixed ladders were spaced more than 14 inches (36 cm) apart, as measured along the ladder's side rails:  a)  At 7943 Oakcrest Place, Montgomery, Alabama:  On or about October 12, 2016 and at times prior; the employer allowed his employees to use a job made ladder to gain access to the roof and the ladder rungs were missing on the first 5 feet of the ladder exposing the employees to a fall hazard of 5 feet to the floor below.
