29 CFR 1910.179(b)(5): The rated load of crane(s) was not plainly marked on each side of the crane:    a) This violation occurred on or about October 13, 2016 in the injection room of the facility located at 3601 East Plano Parkway, Suite 150, Plano, Texas where employees operating an A-Frame, overhead crane with an electrically powered chain hoist to raise and move loads weighing approximately 3,000 pounds, were exposed to a crushed by hazard.     b) This violation occurred on or about October 13, 2016 in the maintenance room of the facility located at 3601 East Plano Parkway, Suite 150, Plano, Texas where employees operating an A-Frame, overhead crane with an electrically powered chain hoist to raise and move loads weighing approximately 3,000 pounds, were exposed to a crushed by hazard.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    a) The employer did not ensure the mixer MMX02 energy control procedures contained:    1. A specific statement of the intended use of the procedure;   2. Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;   3. Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the specific responsibility for them ; and   4. Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.
