Any openings in such partitions or screens were not so sized and located that persons were not likely to come into accidental contact with the live parts or to bring conducting objects into contact with them: On or about 5 Oct 2016 - Employees engaged in the die changed activities on the extrusion machine line 1 were exposed to electrical hazards in that guards were not provided to protect against exposed heating element connections.
