29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.   The employer, TRINITY WELDING AND IRON WORKS LLC., did not submit to the agency documents demonstrating that abatement is complete for each serious violation for which the Agency indicates in the citation that such abatement documentation is required, on our about May 31, 2017. More specifically TRINITY WELDING AND IRON WORKS LLC did not submit to the agency documents demonstrating that the employees working on a controlled decking zone (CDZ) were trained in the nature of the hazards associated with work within a CDZ and the establishment, access, proper installation techniques and work practices required by 1926.760(c) and 1926.754(e).

29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were sitting or climbing on the edge of the basket:  a) Interior of Building - Under Roof Deck: Employees engaged in metal decking activities were stepping on the rail of the articulated boom lift in order to access the roof. On 10/14/16, an employee fell to his death while transitioning between the decking and the lift.  b) Interior of Building - Under Roof Deck: Employee engaged in welding activities was stepping on the rail of an articulated boom lift and exposed to a fall hazard of approximately 5 feet, on or about 10/15/16.

29 CFR 1926.757(e)(5): When construction loads were placed on steel joists, the employer did not ensure that the load was within 1 foot of the bearing surface of the joist end.     Exterior of Building - Center Bay Steel Structure: Bundles of Steel Decking were stacked along the roof beams having every other bundle supported  approximately 4 feet away from the joist end, on or about 10/14/16.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  a) Exterior of the Building - Roof Deck: Employees without utilizing fall protection were engaged in steel erection/metal decking installation. On 10/14/16 an employee fell to his death approximately 45 feet.  b) Exterior of the Building - Roof Deck: Employees were exposed to fall hazards of approximately 45 feet as they transitioned from  the roof structure to the articulated boom lift without the use of a double lanyard. An employee fell to his death on 10/14/16.   Trinity Welding and Iron Works, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.760(a)(1), which was contained in OSHA inspection number 981602, citation number 01, item number 01 and was affirmed as a final order on 1/28/15, with respect to a workplace located at 509 Engle St., Englewood, NJ.  Trinity Welding and Iron Works, LLC was previously cited REPEAT for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.760(a)(1), which was contained in OSHA inspection number 1090230, citation number 02, item number 01 and was affirmed as a final order on 10/13/16, with respect to a workplace located at 1620 Oak Tree Rd., Edison, NJ.
