(a) On or about October 3, 2016, for the employee of a subcontractor under the control of MBA General Contracting, LLC, sustained injuries while performing spray foam insulation work activities when they fell approximately nine feet through a floor cover installed on the roof of the SCADA room hallway of the O&M building at 7687 40th Street, Hebron, North Dakota.
