Westwood Ave. Toledo, OH: The employer did not ensure that machine specific energy control procedures were utilized to control the energization of the Rubber Calender during cleaning task(s). Employees performing cleaning tasks at or near the incoming nip between the bottom and middle roller on the tender side in an energized condition were exposed to crushing and amputation hazards.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Fenner Dunlop Toledo, LLC on or about October 13, 2016, at the address of 146 S. Westwood Ave. Toledo, OH: The employer did not ensure that energy isolating procedures for shutdown were performed prior to performing cleaning tasks on the Calender Machine. Employees performing these task in energized condition were exposed to amputation hazards at or near the machine's point of operation (between roller nip-points), in that energy control shutdown procedures required under 29 CRF 1910.147(d)(2) were not utilized before the employee began the aforementioned work task on the machine.

Westwood Ave. Toledo, OH: The employer did not to ensure that employees who are tasked with cleaning activities on the calender machine were adequately trained in the purpose and skills required for the safe application and use relating to the isolation and control of potential hazardous energy sources. The employees tasked with cleaning activities on the aforementioned machine were not specifically trained to lockout the machine to protect against amputation injuries prior to performing cleaning work at or near the point of rotation (nip-point).

Employees utilizing compressed air in this condition were exposing to flying object hazards and associated debris.
