29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a. On or about 10/14/16 - scrap burning, a respiratory protection program had not been implemented for the employees provided and utilizing a 3M 60621 half-mask respirator during the burning of a dam lock into scrap. The program should address medical evaluation and fit testing. An employee burning on the metal dam structure was exposed to lead at a level of 0.0220 mg/m3. The employee was sampled for 385 minutes during one shift on 10/14/16.
