29 CFR 1926.56(a): General. Construction areas, ramps, runways, corridors, offices, shops, and storage areas shall be lighted to not less than the minimum illumination intensities listed in Table D-3 while any work is in progress:      On October 14, 2016, the northwest stairway was unlit and dark, exposing employees to trip and fall hazards.  The employer did not comply with the requirements set forth in Table D-3 to ensure adequate illumination was provided.       Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:    On October 14, 2016, Miller Concrete Construction, Inc. employees without fall protection were exposed to the fall hazards associated with uncovered floor holes while placing concrete on the third floor.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.    On October 14, 2016, Miller Concrete Construction, Inc. employees without fall protection were exposed to a fall hazard of 17 feet while operating concrete troweling machines near the unprotected west wall opening of the third floor.      Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
