Construction areas, ramps, runways, corridors, offices, shops, and storage areas shall be lighted to not less than the minimum illumination intensities listed in Table D-3 while any work is in progress: On October 14, 2016, the northwest stairway was unlit and dark, exposing employees to trip and fall hazards. The employer did not comply with the requirements set forth in Table D-3 to ensure adequate illumination was provided. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

On October 14, 2016, Miller Concrete Construction, Inc. employees without fall protection were exposed to a fall hazard of 17 feet while operating concrete troweling machines near the unprotected west wall opening of the third floor. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
