29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a)  Front of Building, Second Floor - On or about October 11, 2016, masons were exposed to an 18 foot fall from an unguarded open-sided floor.  b)  First Floor - On or about October 11, 2016, workers were exposed to an 8 foot 7 inch fall  at an unguarded open-sided floor.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around the hole:   a)  Front of Building, First Floor -  On or about October 11, 2016, workers were exposed to an 8 foot 7 inch fall through a 5 foot by 5 foot floor opening which had been left uncovered.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:    a)  Second Floor  - On or about October 11, 2016, workers were exposed to a 20 foot fall from a 38 inch wide wall opening which was not provided with fall protection.  b)  Second Floor  - On or about October 11, 2016, workers were exposed to a 20 foot fall from a 7 foot wide wall opening which was not provided with fall protection.    c )  Second Floor  - On or about October 11, 2016, workers were exposed to an 18 foot fall from a 52 inch wide wall opening which was not provided with fall protection.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(c)(1): When an employee was exposed to falling objects, the employer did not have each employee wear a hard hat and did not erect toe boards, screens, or guardrail systems to prevent objects from falling from higher levels  a)  Front of Building, First Floor - On or about October 11, 2016, laborers delivering building materials into a building were exposed to a struck-by hazard while walking under an active masonry operation and were not wearing hard hats and falling object protection was not provided.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
