29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system. A. Medere Home Improvements worksite located at 5609 Harris Hawk Lane, Sylvania, Ohio: On or about October 14, 2016, the employer failed to ensure employees were protected from fall hazards while working from a ladder jack scaffold. The employee was exposed to a 15.00 foot fall to the level below while installing siding on the East side of the two story residential structure. No form of fall protection was in place.
