29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: A) Medere Home Improvements, conducting work at the site located at 1473 Falcon Cove (Lot 22), Waterville, OH 43566: On or about January 14, 2020, the employer did not ensure that employees were protected from hazards that could result in a head injury. Employees were not using protective hard hats while working below employees who were conducting siding work above. Employees were exposed to potential impact injuries caused by falling materials and/or equipment.

Employees were exposed to approximately 11 feet fall hazard from the roof of the structure to the ground below.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: A) Medere Home Improvements, conducting work at the site located at 1473 Falcon Cove (Lot 22), Waterville, OH 43566: On or about January 14, 2020, the employer did not ensure that employees were protected from fall hazards while working on the ladder jack scaffold without any fall protection in place. Employees, working in this condition, were exposed to approximately 12 feet fall hazard from the scaffold to the ground below. Medere Home Improvements was previously cited for a violation of this Occupational Safety and Health Standard (29 CFR 1926.451(g)(1)(i)), which was contained in OSHA Inspection # 1184458, Citation number 1, Item number 1, and was affirmed as a final order on 12/06/2016, with respect to a workplace located at 5609 Harris Hawk Lane, Sylvania, OH 43560.
