29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  (a) Facility, 2920 Duss Avenue, Ambridge, PA:  On or about October 18, 2016 the exit door behind the mixing area was chained and locked and employees were not aware of the location of the key.    (b) Facility, 2920 Duss Avenue, Ambridge, PA:  On or about October 18, 2016 the exit doors located along the east wall were locked and secured with an additional metal tubing and not able to be opened in the event of an emergency.   The employer, Design Dynamics Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.36(d)(1), which was contained in OSHA inspection number 912327, citation number 1, item number 1a and was affirmed as a final order on November 22, 2013, with respect to a workplace located at 2920 Duss Avenue, Ambridge, PA.

(b) Facility, 2920 Duss Avenue, Ambridge, PA:  On or about October 18, 2016 the exit doors located on the east wall were blocked by sand bags and other materials.   The employer, Design Dynamics Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.37(a)(3), which was contained in OSHA inspection number 812327, citation number 1, item number 1 and was affirmed as a final order on November 22, 2013, with respect to a workplace located at. 2920 Duss Avenue, Ambridge, PA.
