29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about October 13, 2016, and times prior to, in the Tube Mill area, the Tube Winder machines were not effectively guarded to protect employees from reaching inside and contacting the main winding belt and mandrel. Employees were exposed to the hazard of contact with nip points.
