The employee suffered an amputation during this operation.         Table Talk Pies was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1044421, citation number 1, item number 1 and was affirmed as a final order on April 22, 2015, with respect to a workplace located at 120 Washington Street, Worcester, Massachusetts.      Table Talk Pies was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1114302, citation number 1, item number 1 and was affirmed as a final order on March 2, 2016, with respect to a workplace located at 120 Washington Street, Worcester, Massachusetts.
