29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:    Exit H3, Table Talk Pies, 28 Bowditch Drive, Shrewsbury, Massachusetts; On October 19, 2016, Exit H3 and the aisle leading to it were blocked by a powered industrial truck with a barrel attachment.  The powered pallet jack was approximately 48 inches wide and blocked approximately half of the width of the aisle which was marked with yellow lines.

Table Talk Pies, 28 Bowditch Dr., Shrewsbury, Massachusetts; On or before October 19, 2016, electrically rated gloves, Class 1 - 7500 volt gloves, used by the journeyman electrician, had not been replaced or retested every six months as required.  The gloves available to the electrician were dated January 2014.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Table Talk Pies, 28 Bowditch Drive, Shrewsbury, Massachusetts; On and before October 19, 2016, procedures were not developed, documented and utilized for the control of potentially hazardous energy on the Kliklok machine.      a.  Maintenance employees were locking out the Kliklok machine without having or following machine specific lockout procedures.     b.  Employees were clearing jams without locking out the machine.  On or about October 12, 2016 an employee removed a guard to clear a jam without having the machine locked out by an authorized employee.  The employee suffered an amputation during this operation.         Table Talk Pies was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1044421, citation number 1, item number 1 and was affirmed as a final order on April 22, 2015, with respect to a workplace located at 120 Washington Street, Worcester, Massachusetts.      Table Talk Pies was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1114302, citation number 1, item number 1 and was affirmed as a final order on March 2, 2016, with respect to a workplace located at 120 Washington Street, Worcester, Massachusetts.
