Amphenol Thermometrics, Inc., 967 Windfall Rd. St. Marys, PA, Die craft loader area next to Dip solder machine #2595- on or about October 19th, 2016 and times prior thereto: Employees utilized a foot pedal in order to operate a die craft loading machine onsite. The conduit protecting the internal wiring of the foot pedal was disconnected/split exposing the internal wires associated with the foot pedal.

St. Marys, PA, Diode plating area, on or about October 19th, 2016 and times prior thereto: Items including, but not limited to, a chemical spill kit cabinet, and cardboard drum of Mirapic ME-815 were stored directly in front of a 120/240 electrical panel box. The employer did not ensure the area in front of the electrical panel box was kept clear for access.
