29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly: a.) Richardson Cooling Packages LLC in New Castle, PA. On or about October 20, 2016, a roll lifting, forklift attachment was utilized on the Toyota forklift truck. Approval from the Manufactures the forklift attachment was not obtained. In addition, the capacity plate on the forklift was not modified to reflect the effects on the capacity of the forklift when the attachment was used.
