29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems: a) 2355 Snow Road, Semmes AL; On or about October 17, 2016 and at times prior , the employer exposed his employees to fall hazards in that employees were allowed to work on the roof of a new building under construction that had an unprotected edges exposing employees to a fall of up to 16' 2 1/2" to the floor below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) 2355 Snow Road, Semmes AL; On or about October 17, 2016 the employer exposed employees to fall hazards of up to 13' 3" while mounting and dismounting a ladder that did not extend at least 3 feet above the landing surface.
