29 CFR 1910.37(a)(2): Exit routes were not arranged so that employees did not have to travel toward a high hazard area, unless the path of travel was effectively shielded from the high hazard area by suitable partitions or other physical barriers: (a) Production Area; Women's Locker Room - The door leading to the Stamping Department was blocked from use leaving only one exit route door leading to the aisle adjacent to the Plating Department where hazardous chemicals, such as sodium cyanide, were used, on or about October 21, 2016. No abatement certification or documentation required.

MODIFIED DUE TO ISA ON 12/20/16 avd modified to the sign marked not an exiton the door leading from the locker room and into the Stamping Department was removed without managements knowledge on or about October 21, 2016.

29 CFR 1910.120(q)(2)(xi): The emergency response plan did not address, to the extent not addressed elsewhere, personal protective equipment (PPE) and emergency equipment: (a) Tyco Electronics Corporation dba TE Connectivity - The employer's Emergency Response Plan did not include information on the personal protective equipment available for emergency response use, as well as instructions for the use of the personal protective equipment, on or about October 25, 2016. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
